Initial Comprehensive Functional Evaluation
Investment: $750
Your experience begins with an Initial 75-minute Comprehensive Evaluation where we map out your current health, identify imbalances, and design your personalized roadmap for healing. Patients then transition into one of our Signature Functional Medicine Programs below for continued support and lasting transformation.
Programs Include:
- Program Launch Visit: 75-minute Physician Lab Review Visit ( ≈6 weeks after initial evaluation)
- Three (3) physician follow-up visits over the year (30 minutes)
- Two (2) health coaching sessions
- Ongoing nutrition, supplement & lifestyle optimization
- Access to 100+ conventional biomarkers (insurance may apply)
- Discounted specialty testing (GI-MAP, DUTCH, etc.)
- Secure portal messaging for brief check-ins as needed
- Personalized updates and accountability as your health evolves
- Up to $150 of starter supplements

360° Functional Medicine Assessment – “A Healthy You.”
A two-visit deep dive designed to give you clarity and a personalized action plan.
Ideal for patients who are looking to optimize their health.

What’s Included:
- Visit 1 – 60 minutes: This session includes a functional medicine intake, an InBody scan, and a comprehensive evaluation of over 100 functional health labs (some may be covered by insurance) to assess important aspects of health, such as gut and metabolic health, nutrient levels, hormone balance, and inflammation. You will also have access to discounted advanced functional panels (GI MAP, DUTCH, Food Allergy and Sensitivity Testing, etc.)
- Visit 2 – 90 minutes:
Full review of labs with tailored root-cause protocols
Nutrition plan with weekly planner and targeted supplement recommendations. Perfect if you want answers now and plan to continue care with your PCP, or may join a program later. - This two-visit package is ideal for patients without chronic conditions who want a good understanding through a functional lens of their current health.

For Our Current Patients:
To ensure a smooth transition:
1. You may continue with your current care setup with al la carte option until March 30, 2026.
2. After this day, all patients will need to enroll in either a care program or continue al la carte visits with our new pricing structure. Please call our office to discuss what options work best for you.
We appreciate your trust in our practice and are confident that these changes will allow us to serve you even better.
If you have any questions or would like help understanding your options, please do not hesitate to contact our front desk. New pricing will be available on November 1st, 2025.
Please note:
A credit card is required on file at the time of scheduling. If you need to reschedule or cancel, please do so within 48 business hours.
Cancellation Policy:
Program Members:
If you are a member in one of our Signature Programs, you are allowed to change or reschedule your visit, but kindly ask you to give us 72 hours' notice in order for us to accommodate other members/ patients to see Dr. Kapoor in a timely manner.
We do allow one visit to be rescheduled in the event of an emergency or illness if within 72 hours, for our members. Therafter, a cancellation of $50 will apply.
Non Members:
For those patients who are either doing the Two-Visit Functional Medicine Assessment or are continuing care with al la carte visits a 50% cancellation fees apply to all visits cancelled within 72 hours of their scheduled visit ( 3 business days).
You can request a superbill at the end of your visit to submit to your insurance company for any potential reimbursement.
Kapoor will recommend personalized testing during your visit. All testing is optional. The cost of the testing and supplements is not included in the appointment cost.
Dr. Kapoor can see you via telehealth for follow-up visits for those patients living in Pennsylvania. Telehealth appointments are available for reimbursement now as well. Please check with your insurance carrier.
Scheduled visits are required so that Dr. Kapoor can continue to give medical advice. We request that follow-ups be made every 3 to 6 months, depending on the recommendations of Dr. Kapoor. If you are getting prescription medications filled by Dr. Kapoor, a visit every 3 months is required.
